I have the Member Application 2.1.4 and I wanted to know how you can change the current form from 1 to another or even resetting back so you have to re-make it...

I would like to have 2 form which people could use as my clan plays on PC and on XBOX I wanted a form for each.

Is there a way of having 2 forms or because I have messed mine up is there anyway of resetting the application form back so I have to start over?

go to admin panel of MAforms and follow the steps below

the above picture is what MA form will look like when you access the menu page from here you will create your first form by filling out all the fields until you get down the bottom and then hit save changes (remember the "Form Name" will be the overall name of the form and you will see this is the drop down box in between "question list" and "view form" at the top

now you will have created the first form and it will appear in this dropdown box in the picture so to create another one click the arrow to expand the drop down box and select "CREATE NEW FORM" when you select this it will open your second form for you to fill out the same as the first form remembering to keep the "form name" different just repeat these steps and create all the forms you need
