Check the edits in bbcode.tpl and bbcode.php. Make sure those edits are in place and correct.

well i don't know if this will work but i had the same problem and this is how i fixed it. Go to ur forums configuration from admin panel and in the Allowed HTML tags add embed,object,param that did the trick for me. i hope it helps Embarrased
