I know this is an older then heck post, but I also downloaded this module and had the same problems. The fixes are pretty simple though if you still want to try it.

First the clammod_install.php file has a typo, in the second or third line it calls clan_modinstall.php instead of clanmod_install.php, just edit that and the install works.

Also, in the create $createroster line the key field which is roster_id, needs to be set to autoincrement. That's what causes the error duplicate entry 0 for 1 that everyone gets. You can edit that easy enough using phpmyadmin, or adding AUTO after roster_id int(11) I think.
